Kid-Art Contest Permission Form

I GIVE PERMISSION
to my child to submit drawings to OOZ & OZ’s Kid-Art Contest. I understand that if my child’s drawing is chosen for morphing, it will be morphed at no cost to me. I give permission to OOZ & OZ to post the morph on their website as a free download and to display my child’s first name, last initial and age, plus city and state of residence. I am aware that there will be one to four winners each month and that winners will receive a free one-year subscription to OOZ & OZ’s Morph Club. There is no monetary or additional compensation for my child’s drawing. Once this Permission Form is submitted, an email will be sent to me with an attachment of OOZ & OZ’s 5 Easy Rules and Picture Frame page for drawings.
